When a patient and family/caregiver, receive care through one of our affiliate hospices, you can trust that we will have a team of highly trained, hospice focused, professional caregivers who work together to develop a customized plan of care specific to the unique needs of the patient and caregiver/family.

Important elements of hospice care are clinical excellence, compassion, integrity and attention/responsiveness to patients and caregivers. Hospice is all about how well we provide care and services to our patients and their families during the final months, weeks, and days.

We approach care on a holistic, patient-centered basis.  Recognizing that each patient and family deals with illness and grief differently, the hospice care plan is tailored to meet those varied needs, while, at the same time, addressing the symptoms, need for pain management and all other technical aspects of an individual’s care.

The comprehensive plan of care is directed by the patient and carried out by our hospice team, which may include, but is not limited to:

  • Chaplains - Bereavement Counselors
  • Licensed Practical Nurses
  • Home Health Aids
  • Trained Volunteers

All team members work together to ensure a unified care plan that meets every need.  As a patients’s needs change, be assured that the team will adapt to those changes and respond appropriately and timely.  Our team is available 24 hours a day, 7 days a week.

 

 

*Our hospice affiliates, along with 1600 other hospices, partner with an independent researcher to conduct an evaluation of caregiver satisfaction.  Through a comprehensive survey caregivers rate the hospice experience.  The data is collected, aggregated, analyzed and validated by DEYTA, LLC, a third party vendor and leading provider of survey administration services for hospice and home health organizations.
